Frontpage : formulaire

bonjour,
je viens de faire un formulaire sous Frontpage, et une fois que j'ai publier mon site et que je test mon formulaire il ne fonctionne pas. il me met une erreur de page:

Page Web introuvable
HTTP 404
Causes les plus probables :
Il se peut que l’adresse contienne une erreur de frappe.
Si vous avez cliqué sur un lien, il se peut qu’il soit obsolète.

Saisissez à nouveau l’adresse.
